What we do

We coordinate before they leave the hospital. When they walk through the door, we're there.

The hospital called on Thursday to say Dad was being discharged Friday. I panicked — I work, I live an hour away, his apartment isn't set up for someone who just had hip surgery. Haven at Home had someone there when he got home. Medications organized, fridge stocked, someone to help him to the bathroom. They saved us from disaster.

How it works

Call before discharge

The sooner you call, the better we can prepare. Tell us the discharge date (if known), the reason for hospitalization, and what support you think they'll need.

We build a plan

We'll recommend a level of support — PSW, nursing, or both — and schedule the first visit for discharge day or the next day.

Care begins

Your caregiver is there when they get home (or within hours). Medications organized, fridge stocked if needed, someone to help with the first difficult days.

We adjust

The first 72 hours are the most intensive. We scale up or down as recovery progresses. When they're stable, we can reduce hours or hand off to ongoing care.
